THE HOPE FOUNDATION SWLA FUND

The Hope Foundation SWLA Fund was established to assist patients with health insurance, from Southwest Louisiana receive outpatient physical, occupational and speech therapy in order to maximize their recovery.

Eligibility:  

• Applicants must be residents of Allen, Beauregard, Calcasieu, Cameron or Jeff Davis Parish

• Applicants must have had a stroke or brain injury that impacts their ability to functionally walk, talk, eat, or move

• Applicants must be covered by health insurance

Documents required: 

• Copy of the family's most recent federal income tax form 1040 with signature

• Copy of health insurance cards with benefits information

• Written diagnosis and prognosis from physician

• Estimated out-of-pocket expenses
