EMERGENCY RELIEF FUND

COMPETITIVE GRANT OPPORTUNITY

Purpose:  To provide small grants to 501c3 organizations in Southwest Louisiana for the specific purpose of helping them return to their full scale of services in a safe, effective and successful manner. 

Award:  Grants from $500 to $2,500

Deadline to Apply:  May 15, 2020

Notification Date: by May 29, 2020

Grant Application:  Please click on “Apply for Grant” below to create a profile. A link to the application will then be emailed to you. The application shall include the following:

• Organization name/address/phone number/website address

• Executive Director name/phone number/email address

• Website address and/or Facebook page

• Mission Statement

• Amount requested

• Amount of current annual operating budget

• Concise History of Organization – 50 words or less

• Scope of Grant – be brief (less than 200 words) and may be in bullet point or paragraph form and include:

  • Description of what grant will be used for in order to help the organization open safely and successfully; and

  • Timetable for implementation of the grant - preferably in the month of June in order to serve your clients as soon as possible

• Documents to be uploaded

  • IRS Determination Letter stating organization is tax-exempt

  • List of Organizations’s current board members with principal occupations

Reporting Information: A brief update on how the organization used the grant will be required within 30 days of grant award should be emailed to the Community Foundation.  Simple is best!  We want you to help us tell your story to donors, news media, etc. 

• Please include quotes of appreciative staff or clients to help us tell your story.  Update may be in bullet points or not more than 2 paragraphs and include:

  • Description of what grant was used for and how it was implemented

  • Two to five photos showing implementation of grant
